Sudden Cardiac Death in Three Asymptomatic Patients with Brugada Syndrome

Abstract

Brugada syndrome is a channelopathy with characteristic electrocardiogram and probability of syncope or cardiac arrest due to malignant ventricular arrhythmias. Asymptomatic patients, with type 1 electrocardiogram on sodium blockers alone, are at very low risk. Those who have it spontaneously are recommended programmed electrical stimulation for stratification; although this is controversial. The objective was to present three asymptomatic patients with Brugada syndrome and spontaneous type 1 electrocardiogram, who had sudden cardiac death. The spontaneous type 1 electrocardiogram is a risk marker. In some families, the history of having this syndrome or sudden cardiac death should have more value. The negative predictive value of programmed electrical stimulation is not always 100%. The stratification risk of asymptomatic patients with spontaneous type 1 electrocardiogram continues to be a challenge, which must be individualized to decide, together with the patient, the most appropriate conduct.
