Standardized Tomographic Report in the Characterization of Coronary Atherosclerosis

Introduction: In Cuba, ischemic heart disease leads the main unfavorable health indicators. Cardiac tomography modalities are considered the techniques of choice for noninvasive evaluation of coronary atherosclerosis. Structured imaging reporting systems facilitate the understanding and interdisciplinary management of coronary artery disease.
Objective: To determine the relationship between cardiovascular risk factors and the CAC-DRS and CAD-RADS categories in the characterization of coronary artery disease.
Methodology: An analytical, cross-sectional, observational study was conducted during 2019 and 2020. The study population consisted of 273 patients who underwent coronary tomography for suspected coronary artery disease or coronary anomaly and met defined inclusion and exclusion criteria. Clinical variables such as age, sex and cardiovascular risk factors; and imaging variables derived from the CAC-DRS and CAD-RADS classifications were evaluated.
Results: Females predominated, with an average age of 54.39 years. Age showed a strong association with the CAC-DRS and CAD-RADS categories, as did smoking with coronary calcium quantification and the extent of atherosclerotic disease. The expected associations were not found for the rest of the risk factors with imaging variables. Twenty-two patients were incidentally diagnosed with coronary anomalies, in 16 of whom the presence of atheroma plaques was associated.
Conclusions: Positive relationships were found between some of the cardiovascular risk factors and imaging categories dependent on structured coronary tomography reports.
