Abstract

The usefulness of the electrocardiogram in the recognition of cardiopathic patients is unquestionable. Most of the relevant structural heart diseases can present a more or less recognizable electrocardiographic pattern that facilitates their diagnosis. However, there are conditions not directly related to cardiac anatomy or function that can simulate very severe or advanced stages of cardiovascular disease. Among these we can highlight hypokalemia, which in its most severe degrees is capable of producing electrocardiographic alterations similar to those of primary cardiomyopathies or even dangerous ventricular arrhythmias. We describe the case of a patient with severe hypokalemia and an electrocardiogram suggestive of a Brugada type I pattern, with the aim of exemplifying how Brugada phenocopies can produce electrocardiogram patterns very characteristic of the syndrome without actually being so. The patient normalized, once the electrolyte abnormalities were corrected.
