Arterial stiffness in the assessment of cardiovascular risk in the diagnosis of type 2 diabetes

Abstract

Introduction: Diabetes mellitus is a global health problem. It is one of the leading causes of morbidity, disability, and increased risk of mortality from cardiovascular disease.

Objective: To evaluate arterial stiffness as an indicator of cardiovascular risk at the diagnosis of patients with type 2 diabetes.

Methods: A descriptive, cross-sectional study was conducted, including 123 patients over 30 years of age diagnosed with type 2 diabetes mellitus at the José Martí Pérez Polyclinic in Santiago de Cuba province, during the period 2018-2020.

Results: At the time of diagnosis of diabetes mellitus, arterial stiffness was present in 86.2% of patients, with a predominance of moderate arterial stiffness (49.6%). The most affected age group was 51 to 60 years and women. 39.8% were obese, had high blood pressure, and dyslipidemia. A low ankle-brachial index and moderate arterial stiffness were present in 43.9% of cases. Vascular wall abnormalities were detected, primarily increased intima-media thickness (69%) and an increased carotid resistance index (63.4%). Very high cardiovascular risk predominated in 42.3% of diabetics, suggesting a statistically significant relationship between arterial stiffness and cardiovascular risk.

Conclusions: In the initial diagnosis of type 2 diabetics, arterial stiffness estimated using photoplethysmography allows cardiovascular risk to be determined.
