Predictive scales of in-hospital death in acute myocardial infarction

Abstract

Introduction: The use of the in-hospital death prediction scores in patients with acute myocardial infarction is limited in clinical practice.
Objective: To characterize the internationally developed predictive score for in-hospital death due to acute myocardial infarction in relation to the variables included, their creation and validation process, and their potential feasibility for implementation in low-and middle-income countries.


Methods: A review of articles was carried out in six search strategy: 1) in-hospital death, 2) scale or predictive model and 3) acute myocardial infarction. After analyzing the inclusion and exclusion criteria, 15 articles whose objective was the prediction of in-hospital death in patients with acute myocardial infarction were reviewed.

Results: 66,7% of the score were created from multicenter clinical registries. Seven scores (46,7%) included both types of infarcts. The average number of predictors included in the scores was seven. The most commonly used statistical technique was multivariable logistic regression (86,7%). Killip Kimball class and age were the most frequent predictors included and were present in 73,3% of the scores. Sixty percent of the scores included variables that are difficult to measure in low-and middle-income countries.
Conclusions: Most in-hospital death prediction scores were designed in high-income countries and include variables that are difficult to measure in low-and middle-income countries like Cuba.
