Disfagia lusoria en adultos, un reto de la cirugía cardiovascular

Resumen

La arteria subclavia derecha aberrante constituye una variedad anatómica frecuente dentro de los anillos vasculares. Se presenta  un paciente de 44 años de edad y 48 Kg de peso, con pérdida de peso y  disfagia. Se observó en el esofagograma con bario el  estrechamiento de la luz esofágica en la  pared posterior, además en la angiografía y el angiotac se evidenció la presencia de esta anomalía. Se realizó cirugía por esternotomía parcial, se disecó la arteria subclavia derecha aberrante y se seccionó cerca de su  origen, el cabo distal se suturó y el proximal se anastomosó a la carótida derecha con sutura de polipropileno 5/0 terminolateral. Tuvo un posoperatorio sin contratiempos y la estancia en el hospital de 5 días. Evolucionó sin alteraciones a los seis  meses de evolución.

The right aberrant subclavian artery constituted a frequent form of vascular rings. This is a case of a 44 year old male patient and 48 Kg of body weight with personal history of failure to thrive.  A posterior narrowing of the esophagus lumen was noted in the barium meal and the CT scan confirmed the presence of the disease.A partial sternotomy was made, the aberrant artery was dissected and sectioned nearby his origin, the distal stump was closed and the proximal one was anastomosed to the lateral aspect of the right carotid artery. There was no operative or late morbidity or death. Remains free of complications 6 months after surgery.
